PERMIT #: M- 2000 -092 <br />INSPECTOR'S INITIALS: TCI <br />INSPECTION DATE: March 13, 2013 <br />OBSERVATIONS <br />This inspection was conducted as part of the regular monitoring program. This inspector met with Dave <br />Johnston (City of Walsenburg) in the City office prior to the inspection. The Operator was represented by Butch <br />Corsentino during the inspection. The Walsenburg Gravel Pit is a 110 construction materials type permit <br />located approximately three miles northeast of Walsenburg, off State Highway 10 in Huerfano County. <br />Inspection: <br />The mine entrance sign displayed the mine reclamation permit ID number (see Photo 1), but no Operator name <br />was displayed. All five permit boundary corners were marked with T -posts and /or pvc pipe (see Photos 2 and <br />3). Additional T -posts were positioned to provide line of sight boundary markers. This is a terrace mine <br />operation. It was not active at the time of inspection. <br />Relatively short highwalls (approximately 8 feet in height) were observed and appeared stable (see Photo 4). <br />Two small material stockpiles were observed (see Photo 5). The pit floor has fairly well established vegetation, <br />suggesting the pit has not been very active. <br />No noxious weeds or erosion features were observed <br />approved mine plan. <br />File Review Notes: <br />• The total permitted area is 9.9 acres. <br />The operation appeared to be in compliance with the <br />• The previous inspection was performed on 10/23/2008. Two problems were cited: 1) for being inactive <br />for more than 180 days, and 2) for not posting a permit sign. Both problems were resolved. (Note: the <br />letter received by the Division on 12/22/2008 showing the permit sign stated the Operator would add the <br />Operator name to the sign as soon as weather permitted. As Photo 1 shows, this has not yet occurred). <br />• The annual fees and report for 2012 (12/14 anniversary date) was late at the time of the inspection, but <br />was received by the Division on 4/24/2013. <br />• The 2012 annual report indicates the latest mining activity was performed in August of 2012. Based on <br />observations made during the inspection the stated infrequent use, the Division believes the operation <br />should be in intermittent status. <br />Current Stipulations: <br />None. <br />Summary and Recommendations: <br />The Division recommends the following tasks be completed by the Operator: <br />1. Due to the infrequent use of the pit, the Operator should apply to the Division for Intermittent Status (IS) <br />within the next year (10/25/2014) by submitting a Technical Revision and the requisite $216 fee. <br />2. The Operator should post the Operator's name on or below the permit sign on or before November 30, <br />2013. <br />Page 2 of 4 <br />
