Grandview Dr. <br />Tempe, AZ 85281 <br />Sept. 1, 2013 <br />Division of Reclamation, Mining, and Safety <br />1313 Sherman St., Room 215 <br />Denver, Colorado 80203 <br />Annual Report: Deep Creek Placer (Permit No. M 1986 -102) <br />Dear Sir or Madam: Please accept this Annual Report on the "Deep Creek Placer," a temporarily <br />inactive limestone mine 7 miles west of Telluride, Colorado at the confluence of Dee ek and <br />the San Miguel river. <br />- -No mining has taken place in the year Sept. 1, 201SL— present SEP 2013 <br />-- Portals to the mine have been checked and they are secure <br />-- Equipment room entrance locked and secured. (-�DMSIONOFR_=_, <br />-- Routine maintenance of access road carried out, summer 2013 MININGANL <br />-- Annual maintenance of stormwater "ponds" along access road for capturing runoff has been <br />carried out. <br />-- Access road was cleared and graded. <br />-- Access road gate is secure and locked <br />- -Bond continues to be sufficient for reclamation (if necessary) of mine site. <br />- -No disturbances to the surrounding land or the mine itself are anticipated. <br />-- Regular maintenance of roads, equipment apron, portals, and stormwater ponds will be <br />conducted by myself over the course of the coming year. <br />i <br />74F Please accept this statement as "nofice of Temporary Cessation" as per description in your letter <br />dated February 28, 2013. As per the requirements outlined in that letter, I have included the <br />additional fee and statements: <br />1. Enclosed is check for $144.00 for revision fee. <br />2. Initial five -year period for TC will commence on our anniversary date of Sept. 4, 2013 <br />3. Reason for non - production: <br />While there are on -going discussions in the family regarding future uses of the mine, the <br />cost of fuel and other factors such as the demand for limestone in the marketplace make it <br />difficult to predict what commercially viable opportunities will present themselves in the <br />near future. We are requesting that the mine continue under "Temporary Cessation" <br />status for the next five years. As there are very few commercially accessible limestone <br />sources within a 100 mile radius of the mine, we feel the mine has future potential as a <br />source for high quality calcium carbonate. <br />4. A general plan for the resumption of mining: <br />The resumption of mining would require the following (minimal) steps: <br />a. Clean -up of non - mining related storage. <br />b.
