and in effect as of June 7, 2010. Page 19 of the 2012
AHR identifies the NPDES permit as CO- 0000221, this is
incorrect. The correct permit associated with outfalls
002, 003, and 004 is CO- 0048275.
The selenium standard was exceeded a total of four
times at two NPDES outfalls during the water year. A
Notice of Violation /Cease and Desist Order, Number: 10-
120912-1 was issued by CDPHE's WCQCD at the Peabody
Sage Creek Mine on September 12, 2012.
