The statements in the Application are true. <br /> <br />2. The name of the stream involved is Supply Creek. <br /> <br />3. The name and address of the Claimant: Colorado Water Conservation -- <br />Board; 1313 Sherman Street, Suite 721; Denver, Colorado 80203. <br /> <br />v <br /> <br />4. The source of the water is Supply Creek, tributary to the North Fork <br />Co 1 orado River. <br /> <br />5. Description of beginning and end points of minimum stream flow <br />claimed: The natural stream channel from the confluence of the North and. <br />Middle Supply Creeks in the SW1/4SW1/4 of Section 26, T. 4 N., R. 76 W. of the <br />6th P.M.c at Lat. 40016'25" N., Long. 105052'46" W. as the upstream terminus, <br />and extending to the headgate of the Redtop Valley Ditch in the SE1/4SW1/4 of <br />Section 2, T. 3 N., R. 76 W. of the 6th P.M. as the downstream terminus. <br /> <br />'1',~",~,,,,. <br />t'ii'_'f".~,-,.-.1..
