the use of the awlicant' s portion of =nserved water.

(A) When the right is oot leased, reserved instream or dedicated
to instream use, the certificate shall in:::lude:

(i) 'Il1e name ani aQjress of the water right holder; ,
(ii) 'Il1e decree ani page number or the certificate number of
the original water right;
(iii)'Il1e location of the point of diversion:
(iv) 'Il1e sooroe of water;
(v) 'The priority elate of the right;
(vi) 'Il1e rate at ~dl water may be diverted ard the arramt
that may be diverted;
(vii)'The p.n:pose for which the water is used.
(viii) 'Il1e cl.es=iption of the legal sub:livisions of lard to
..roch the water is ~.
(ix) Any terms or oorxtitions fran the original water right

(x) Any other special ei=Dnst:.an:les or limitations affectirg
the use of water.

(B) When the conserved water right is leased by the water right
holder, a certificate shall be issued:

(i) Subject to the terms an:l. corxlitions of the lease.
(ii) eont.ainin;J the same information listed in (i) thro.lgh
(x) above.
(iii)Describirq the place of use the right will revert to
upon the expiration of the lease.

(e) When the conserved water right is reserved instream for
future cut of stream use, a certificate shall be issued
which states:

(i) 'Il1e name am aQjress of the water right holder;
(ii) 'Il1e decree am page fIITmhr>..r or the certificate number of
the original water right;
(iii)'Il1e = of water:
(iv) 'Il1e priority elate of the right;
(v) 'The rate am duty whidl will be managed instream;
(vi) 'The strealIl reach tI1rtn;Jh which the conserved water will
be managed;
(vii)Any terms or corxlitions frc:m the original water right
wnidl still awly.
(viii) Any other special eircumstan::.es or limitations
affecting the use of water.

(e) When a conserved water right is dedicated to instream use a re,;
