<br />REPRESENTATIONS AND CERTIFICATIONS. <br /> <br />The proposer represents and certifies as part of his proposal that: <br />(check or complete all applicable boxes or blocks and return with <br />proposal) . <br /> <br />EQUAL OPPORTUNITY.
