This form must be completed and returned to us as soon as <br />the practice has been performed, if application for cost-sharing is <br />to be made~ <br /> <br />The cost-sharing approved for the practice shown on the attached form <br />AC~245 may not be chang~d to another practice without the prior <br />approval of the county ASC committee. <br /> <br />The following items will serve as a guide in completing and reporting <br />the approved practice: <br /> <br />1 Carry out the practice as soon as practical in accordance with the <br />requirements upon which cost-sharing is conditioned. These require- <br />ments are intended to insure an effective and successful )ob so th.Jt <br />th~ practice will accomplish the conservation objective. Specifica- <br />tions which must be met to qualify for cost-share (s) approved for <br />the practice are to be obtained from SCS. <br /> <br />2 Furnish this office a report of performance on the attached ACP-245 <br />immediately upon completion of the practice and not later than the <br />date indicated on the form; otherwise, the approval for cost- <br />sharing will be cancelled. <br /> <br />3 If the practice cannot be completed by the performance date indicated <br />on the enclosed ACP-245 for raasons beyond your control, you may <br />request additional time to complete the practice. Such request must <br />be furnished to this office jn writing prior to the performance <br />reporting date. <br /> <br />4 You are requested to notify the county office at once if you deter- <br />mine not to perform the prjctice. By so doing, the funds can be <br />made available for other higD priority and needed conservation in <br />the county. <br /> <br />5 Keep any sales slip, weight, invoice, or other evidence for <br />material cost, etc. used in connection with each practice. They <br />will be used by this office in determining your cost-share amount. <br />If your own labor or equipment is used in completing the practice, <br />a complete itemized statement must be submitted by you. Such state- <br />ment must be documented by date, rate per YOUT, and number of 120urs <br />per day. <br /> <br />Over <br /> <br />/ <br />
