The statements in the application are true. <br /> <br />2. The name of the stream involved is Middle Fork Williams <br />Fork River. <br /> <br />3. The name of th.. claimant and address is The Colorado <br />Water Conservation Board; 823 State Centennial Building; 1313 <br />Sherman Street; Denyer, Colorado. <br /> <br />4. The source of the water is Middle Fork Williams Fork <br />River, tributary to Williams Fork River, tributary to the Colo- <br />rado River. <br /> <br />5. Descriptio~ of beginning and end points of minimum <br />stream flow claimed: the natural stream channel from a point <br />located at Lat. 39046'34" N.; Long. 106000'13" W. as the upstream <br />terminus, and its confluence with the Williams FOrK River at Lat. <br />39046'50" 1'1.; Long. 106.00'18" W. as the downstream terminus. <br /> <br />6.
