The May 14 LOMR revised the
effective Flood Insurance Rate Map (FIRM) and Flood Insurance Study (FIS) report for your community
to show the effects of the Kohls project along City Park Channel from Sheridan Boulevard to
approximately 1,400 feet upstream. Because the effective date ofthat LOMR was later then the effective
date (August 18, 2004) of the new countywide FIRM and FIS report for your community, which was being
processeliafilie time ofllie revision request, the updated map panel number, profile panel number, and
vertical datum were used for the annotated FIRM and FIS report included with the May 14 LOMR.
However, the community name and FIRM panel number were incorrectly shown on the annotated FIRM
panel attached to that LOMR. The purpose of this LOMR is to show the proper community name and
panel number on the annotated FIRM panel for the May 14 LOMR.

We have revised the FIRM panel from the May 14 LOMR to reflect the correct community name and
FIRM panel number. The flood hazard information shown on the annotated FIRM panel and profile panel
enclosed with the May 14 LOMR has not changed. The corrected community name and panel number are
shown on the enclosed annotated copy of FIRM Panel 0091 F. For your reference, we also have enclosed
a copy of the Detennination Document and an annotated copy of Profile Panel 07P that was transmitted
with the May 14 LOMR. This LOMR is effective as of the date shown above.

The May 14 LOMR also affected the City of Westminster, Colorado. This LOMR has not revised the
flood hazard information shown on the annotated FIRM panel and profile panel enclosed with the May 14
LOMR for the City of Westminster.

A review of the determination made by this LOMR and any requests to alter this detennination should be
made within 30 days. Any request to alter the detennination must be based on scientific or technical data.

Because this LOMR will not be printed and distributed to primary users, such as local insurance agents and
mortgage lenders, your community will serve as a repository for these new data. We encourage you to
disseminate the information reflected by this LOMR throughout the community, so that interested persons,
such as property owners, local insurance agents, and mortgage lenders, may benefit from the information.
We also encourage you to prepare a related article for publication in your community's local newspaper.

~;;>
