Mountain Ave., Fort Collins, CO 80521 (970) 498-5722 Colorado county Phone Number: Type of Entity: Section 2 - Project Description The Project is a flood mitigation feasibility study for the area along Dry Creek in the City of Fort Collins and adjacent areas of Larimer County, Colorado, The purposes of the study are to define specific mitigation measures to lessen flood severity and reduce future flood damages along certain developed reaches of Dry Creek, SECTION 3 - TIME FOR PERFORMANCE Project Beginning: Upon the effective date of this contract Project Finish: One (1) year from the effective date of this contract SECTION 4 - DISBURSEMENT SCHEDULE The STATE shall disburse funds to the CONTRACTOR to pay for the costs associated with the Project up to a maximum amount of $50,000 in the following manner: a. Statement of costs, During the completion of the tasks, the CONTRACTOR shall prepare statements of the costs of the Project and submit said statements to the STATE for payment. Each statement shall indicate the tasks that have been performed and the percentage completion of each task by task number as specified in the Scope of Work, and shall include all supporting technicai documents as specified in the Scope of Work, b. Payment of statements, Upon receipt and approval by the STATE of each such statement of costs, the STATE will, within 30 days from the approval of such statement, pay to the CONTRACTOR the amount of the statement or such portion thereof as has been approved by the STATE, Appendix 1 Contract C150080
