<br />Therefore, any individual property owner, <br />developer, or other private party who wishes to <br />request a Conditional Map Revision should <br />submit the request to the CEO or the designated <br />community official. <br /> <br />The CEO or the designated official should review <br />the request and, when forwarding it to the <br />FEMA Regional Office, should state whether the <br />community supports it. When FEMA receives a <br />conditional map revision request directly from a <br />private party, without community concurrence, <br />FEMA will ask the requestor to provide evidence <br />that the CEO or designated official is aware of <br />the proposed modifications on which the <br />request is based. <br /> <br />Requests for Conditional Map Revisions should <br />be submitted to the appropriate FEMA Regional <br />Office. The Regional Office will review the <br />request and forward it to the FEMA <br />Headquarters Office in Washington, D.C., for <br />disposition The FEMA Regional Offices and <br />their addresses are listed in Appendix D. <br /> <br />Required Supporting Data and <br />General Technical Guidance <br /> <br />The types and amounts of technical supporting <br />data that must be submitted in support of a <br />request for a Conditional Map Revision are <br />essentially the same as the supporting data <br />described for Map Revisions in Chapter 4. <br />Therefore, to identify the technical supporting <br />data that must be submitted, you should refer to <br />Chapter 4--particularly, the sections entitled <br />"Requ;Tetl SuppoTting Data" ana "GeneTal <br />Technical Guidance." <br /> <br />The main difference between the types of <br />supporting data required for Map Revisions and <br />Conditional Map Revisions is that any maps, <br />plans, drawings, measurements, or ground <br />elevation data submitted in support of a request <br />for a conditional map revision will not reflect <br />existing conditions and consequently cannot be <br />certified "as-built." In addition, the operations <br />and maintenance plans that are to be formally <br />adopted by a community, which are part of the <br />required supporting information for a Map <br />Revision request based on the effects of a levee <br />or other flood protection system, are not <br /> <br />, <br /> <br />required when a request is made for a <br />Conditional Map Revision. <br /> <br />All data submitted in support of a request for a <br />Conditional Map Revision must, however, reflect <br />final design conditions. The requestor should <br />not interpret the lack of a requirement for <br />certified as-built supporting data to mean that <br />incomplete data and vague descriptions of <br />proposed projects will provide FEMA with an <br />adequate basis for a conditional determination. <br />In addition, although an as built certification <br />would not apply to design plans and other <br />supporting data for a Conditional Map Revision, <br />all data submitted must still be stamped and <br />signed with the seal of a registered professional <br />engineer or licensed land surveyor, as <br />appropriate. <br /> <br />As noted previously, fees are charged for the <br />review of conditional map revision requests. <br />The initial fee, which FEMA must receive before <br />it will review the request, varies according to the <br />nature of the proposed modifications. Specific <br />fees have been established and are listed in <br />Section 72.3 of the NFIP regulations for the <br />following types of proposed modifications: <br /> <br />. Alteration of topography for single lots <br />and single structures <br /> <br />. Alteration of topography for multiple <br />lots and multiple structures <br /> <br />. New bridge or culvert (no <br />channelization) <br /> <br />. Channel mooi{ication onl~ <br /> <br />. Channel modification and new bridge or <br />cu I vert <br /> <br />. Levee, berm, or other structural measu re <br /> <br />. Structural measures on alluvial fans <br /> <br />For requests based on a combination of <br />proposed modifications, the initial fee will be <br />determined by the one modification, out of <br />those proposed, for which the highest fee has <br />been established.
