TERMS AND CONDITIONS OF PERMIT ([(Int.) <br /> <br />G. Inactivation Notice <br /> <br />When a site has been finally srabiliz,"d in accordance with the SJ.l.MP, the operator of the facility shall submit a <br />Inactivation Notice :hat is signed in accordance with Pan I.E. I. of the permit. The content of the Inactivation <br />Notice is described in Part 1.11.5. ,of the permit. A copy of the Inactivation Notice form will be mailed to the <br />permittee along with the permit ce't(i'ication. Additional copies are availabl,' from the Division. <br /> <br />The description of ':Iinal stabi'izat;O/, .. at Pan I.B.4 of the permit has been erpanded to include a narrative <br />description, as well .IS a numeric crirerion. This should provide permittees with more flexibility in inactivating their <br />cenification. <br /> <br />II. Transfer of Pennit <br /> <br />When responsibility for storm water d,scharges at a construction site changes from one individual to another, the <br />permit shall be transferred in acco.'dance with Part I.A.6. of the permit. The content oj the Notice of Transfer and <br />Acceptance of Terms of a Constructiun General Stormwater Discharge Permit is described at Pan IA.6. of the <br />permit. The Notice of Transfe' form will be mailed to the permittee along wirh the pennit certification. Additional <br />copies are available _ From the Divi.sioi'l. <br /> <br />I. Duration of Pennit <br /> <br />The general pennit "ill expire on /UM 30, 2002. The permittee's authority to discharge under this permit is <br />approved until the expiration date of the general permit. Many permittees will have filed an Inactivation Notice <br />(see section G. above) by that time a'ul will not be affected by permit expiration. Any permittee desiring continued <br />coverage under (he general permit m:tst apply for recenification under the general pennit at least 90 days prior to <br />its expiration date. <br /> <br />Daniel Beley <br />February 19, 1997 <br />
