MT -2 Form 6, Part 2 - Channel Description

The channel inlet and outlet, as well as a majority of the reach within this project will not be changed, A pond will be graded
into the chafUlel upstream of station 3580 to help reduce velocities within the creek. The channel shape and materials will be
the same as the existing channel.

MT-2 Form 6, Part 4 - Drawing Checklist

The details of the pond are shown in the topographic workrnap. The inlet and outlet of the channel will not change, there will
be no accessory structures, the channel lining will not change, and the channel cross sections will not change.

MT -2 Form 6, Part 5 - Hydraulic Considerations

The channel was not redesigned, therefore this section is not applicable. Any changes in the cross section at the location of the
pond are covered in the Hec-2 Hydraulic model for the proposed conditions,
