<br />Appendix 2 <br /> <br />Flood Insurance study Data Request Fora <br /> <br />FLOOD INSURANCE STUDY IFIS} DATA REOUESTS <br /> <br />Requests for FIS data should be made in writina to: <br /> <br />Reaions I-V <br /> <br />Reaions VI-X <br /> <br />Flood Insurance Information <br />Specialist <br />c/o Dewberry & Davis <br />2953 Prosperity Avenue <br />Fairfax, Virginia 22031 <br />FAX: (703) 876-0073 <br /> <br />FEMA Project Library <br />c/o Michael Baker, Jr., Inc. <br />3601 Eisenhower Avenue <br />Suite 600 <br />Alexandria, Virginia 22304 <br />FAX: (703) 960-9125 <br /> <br />The following information should be included in your written request: <br />Complete community name (including county) <br /> <br />Community Identification Number <br /> <br />Name(s) of flooding source(s) and specific location(s) for which <br />data are needed <br /> <br />Specific data needed: <br /> <br />HEC-2 input and output files <br />Topographic data <br />etc. <br /> <br />Effective date of FIRM/FBFM for which data are requested (enclose an <br />annotated copy of FIRM/FBFM if available identifying area of interest) <br /> <br />Agreement to pay costs associated with processing the request <br /> <br />Fee limit after which authorization is needed <br /> <br />Contact person's name, address, and phone number <br /> <br />The average request takes approximately 2 to 4 weeks to fill and may cost <br />between $100 to $200. <br /> <br />You will be contacted after we have determined if the data are available and <br />the cost to fill the request has been determined. <br /> <br />Do not include payment with your request letter. <br /> <br />Checks or money orders should be made payable to the National Flood Insurance <br />Program and sent to: <br /> <br />Federal Emergency Management Agency <br />Fee Collection System <br />P.O. Box 398 <br />Merrifield, Virginia 22116 <br /> <br />Data will be released upon receipt of payment. <br /> <br />A2-1 <br />
