<br />Rule 5. Reauirements for Construction. or EnlarGement. of <br />Jurisdictional Dams or Reservoirs: <br /> <br />Rule S.A. An owner proposing 'to construct or enlarge a juris- <br />dictional dam or reservoir shall submit an application package, In a <br />form acceptable to the State Engineer, and receive approval of the <br />construction plans and specifications from the State Engineer prior to <br />commencing construction. The application package shall be prepared by <br />an engineer, and shall consist of the following: <br /> <br />1. Application Form <br />2. Construction Plans <br />3. Construction Specifications <br />4. Classification Report <br />5. Hydrology Report <br />6. Geotechnical Report <br />7. Design Report <br />8. Instrumentation Plan <br />g. Cost Estimate <br />10. filing Fee <br /> <br />The requirements for each of these items are as follows: <br /> <br />S.A.(I) A completed application form provided by the State <br />Engineer. This form will be the only information normally available <br />to the public before the project is approved for constructign. <br /> <br />5.A.(2) Construction drawings or plans which shall meet the <br />following requirements: <br /> <br />5.A.(2)(a) The plans shall show the design of the dam and each <br />appurtenant structure in sufficient detail so that the contractor or <br />builderis able to construct the proposed structure from the plans and <br />the specifications (see 5.A.[3][a]). <br /> <br />S.A.(2)(b) The first sheet shall include, as a minimum, the <br />name of the dam; the county; Water Division and former Water District <br />In which the dam is located; a list of the drawings that follow the <br />cover sheet; the design engineer's seal (crimp type not acceptable) <br />and signature; the State Engineer's statement and engineer's <br />AS-CONSTRUCTED statement located in the lower right quadrant of the <br />drawing both in the form as follows: <br /> <br />Approved on the <br /> <br />State Engineer <br />By: <br />Deputy <br /> <br />day of <br /> <br />19_. <br /> <br />- 10 - <br />
