The estimated total cost of
the project is $400,000. Please see the attached project summary and map.

Backaround
The LAWA, located in Weld County, operates the Lake Arrowhead Reservoir for storage of
irrigation water used by property owners who live in the Lake Arrowhead subdivision. The
reservoir is located 2 miles southwest of Greeley on Ashcroft Draw, a tributary to the South Platte
River. The dam, constructed in 1963, is a Class 3 structure, 30 feet high, impounding a reservoir
of 200 ac-ft.

On August 10, 2002 a large sinkhole was discovered on the downstream slope of the dam, over
the outlet pipe. ihe sinkhole, about 15 feet in diameter and 20 feet deep, was caused by the
failure of the 24 inch corrugated metal outlet conduit. The State Engineer's Office recommended
the lake be drained until repairs could be made. The LAWA closed the road that crosses the dam
and began pumping the reservoir down. They also started investigations into the cause of the
failure and the best method to repair the dam.

The LAWA needs to rehabilitate the outlet on the Lake Arrowhead Dam so that the reservoir can
be placed back in service and so that the county road across the dam crest can be reopened.
Work is tentatively scheduled for completion in fall-winter 2002.

Feasibilitv Studies
John Gauthiere, P.E. has completed an initial assessment of alternatives available for
rehabilitation of the dam. LAWA and John Glluthiere are completing the feasibility study in
accordance with CWCS guidelines, to include an assessment of alternatives available fo~
rehabilitation of the Lake Arrowhead Dam.
