3 rights - plans for augmentation. (2) (a.) The water judges ofthe various

4 divisions shall jointly prepare and supply to the water clerks standard

5 forms which shall be used for such applications and statements of

6 opposition. These forms shall designate the information to be supplied

7 and may be modified from time to time. Supplemental material may be

8 submitted with any form. ALL FORMS SHALL REQUIRE THE APPLICANT TO

9 INDICATE WHETHER THE APPLICANT IS EITHER THE OWNER OF LAND

10 OVERLYING GROUND WATER IN THE DAWSON, DENVER, ARAPAHOE, OR

II LARAMIE-Fox HILLS AQUIFERS, OR IS A MUNICIPAL OR QUASI-MUNICIPAL

12 WATER SUPPLIER THAT MAY ADOPT A DEEMED CONSENT ORDINANCE OR

13 RESOLUTION PURSUANT TO SECTION 37-90-137 (8). In the case of

14

applications for a determination of a 0ater right or a conditional water

15 right, the forms shall require, among other things, a legal description of

16 the diversion or proposed diversion, a description of the source of the

17 water, the date of the initiation of: the appropriation or proposed

18 appropriation, the amount of water cla~med, and the use or proposed use

19 of the water. In the case of application~ for approval of a change of water

20 right or plan for augmentation, the 'forms shall require a complete

21 statement of such change or plan, including a description of all water

22 rights to be established or changed QY the plan, a map showing the

23 approximate location of historic us~ of the rights, and records or

24 summaries of records of actual diverSions of each right the applicant

25 intends to rely on to the extent such records exist. In the case of

26 applications that will require construction of a well, other than
