Scope of Services <br /> <br />The Project consists of the Contractor establishing a water conservation and xcriscape resource section in the Berthoud Library. Grant funds will be <br />used for the purchase of books and literature and for publicizing of the new section located at the library. <br /> <br />The goal of the Project is to provide a resource section at a public library that is accessible to the residents of the community and surrounding areas. <br /> <br />The Contractor will coordinate the Project and work directly with library starf to obtain the necessary resources. The Contractor will keep track of <br />usage and geographical location ofresidenls using the resourt:c section. The Contractor will conduct a telephone survey of district customers <br />concerning knowledge and usage of the resources section. A compilation of survey results will be completed. The Contractor will promote and <br />publicize the resource section through their own newsletter and in cooperation with the local newspaper and television station. <br /> <br />A Final Report shall be compiled upon completion of the Project. The Final Report shall include a summary of the Project, discussion of the Project <br />implementation and Project findings. The Final Report will not only conclude the Project but will serve as information to be shared with other <br />Colorado communities. <br /> <br />2. Proiect Time of Performance <br /> <br />TIle Project will commence on May I, 1997 or upon full and proper execution of this Contract. <br /> <br />The Project will be completed on or before February 27, t 998. However, the Project time of perfonnance may be extended subject <br />to the mutual agreement of the State and Contractor. A written request for extension shall be submitted to the State by lhe <br />Contractor at least thirty (30) days prior to February 27, 1998, and shall include a full juslificalion for the extension request. <br /> <br />3. Proiect Fundinl! Schedule <br /> <br />a. GRANT Agreement: <br /> <br />I. <br /> <br />Initial grant payment to be made within thirty (30) days of the date <br />of execution of this contract by the State Controller or his <br />designee. <br /> <br />$4,500 <br /> <br />II. <br /> <br />Final payment to be made only upon acceptance of Project <br />completion. The Contractor will submit a final financial statement <br />documenting the expenditure of all Grant funds for which payment <br />has been requested and a final narrative report on the Project <br />implementation and findings. <br /> <br />$500 <br /> <br />TOTAL $5,000 <br /> <br />All requests for payment after the first payment will be initiated by the Contractor in accordance with the provisions in <br />Paragraph 5 of the main body of this Contract. <br /> <br />5. Contrad Monitorinl! <br /> <br />The Department of Natural Resources, Co.lorado. WaterCo.nservatio.n Board, will monitor the Project cn an as-needed basis. <br /> <br />6. Reportine: Schedule <br /> <br />The Contractor will submit financial and narrative status reports detailing Project progress and properly documenting all to.- <br />date expenditures of Grant funds at the time each payment request is made, in acco.rdance with the payment schedule. <br /> <br />Page I of I Page <br />
