Ph 2 Grant - Exhibit A <br /> Statement Of Work <br /> Prepared Date: April 2,2018 <br /> Name of Grantee: San Luis Valley Irrigation District <br /> Name of Water Project: Rio Grande Reservoir Rehabilitation—Phase 2 <br /> Funding Source: SB12S-002& HB17-1248 appropriation—Construction Fund <br /> Water Project Overview: <br /> This Project is a continuation of a previous project phase and therefore called Phase 2. It will include <br /> the rehabilitation of the Reservoir's outlet. This includes constructing a new concrete intake structure <br /> with new guard gates and the lining of the intake tunnel. In addition the construction of a new outlet <br /> works including: a reinforced lined bypass tunnel; steel lining of the existing outlet tunnel; a ten-foot <br /> diameter steel pipe to extend the outlet 150 feet; twin 84-inch fixed cone valves and a 36-inch low <br /> flow valve. <br /> Project Objectives: <br /> The purpose of the Project is to provide operational efficiencies and improve stream health and <br /> utilization of Rio Grande water by rehabilitating the outlet works of the on-channel Rio Grande <br /> Reservoir Dam. The Reservoir has a storage right of 51,113 acre-feet and delivers water to nearly <br /> 62,000 acres of agricultural land in the San Luis Valley. The Reservoir's outlet has long been a limiting <br /> factor in the administration of the Rio Grande. <br /> Tasks <br /> Task 1 —Project Management <br /> Description of Task: <br /> This task includes : <br /> a) Onsite Construction Management, <br /> b) Project Oversight, <br /> c) Public Outreach, <br /> d) Permitting Coordination <br /> Method/Procedure: <br /> a) Site coordination <br /> b) Daily progress review, Coordination with District reservoir storage requirements <br /> c) Press and Shareholder communications <br /> d) Coordination with existing and future permit requirements <br /> Deliverable: <br /> A final construction report summarizing the project shall be provided to the CWCB. <br /> Construction Fund Grant- Exhibit A- Statement of Work Page 1 of 3 <br />
