Effective April 22, 2016,WPX Energy Rocky Mountain, LLC changed its name to TEP Rocky
Mountain LLC("TEP"). TEP's address is 4828 Loop Central Drive,Suite 900, Houston,Texas 77081. On
behalf of TEP, Davis Graham &Stubbs LLP("DGS") submits this Notice of Name Change to the Colorado
Water Conservation Board (the "Board")for recognition of the change of name and change of address
as to that certain Oil and Gas Lease dated March 10, 2010, and recorded March 24, 2010, at Reception
No. 783697, of the official public records of Garfield County,Colorado (the "Lease"). The Lease covers a
parcel of land located in Section 7,Township 7 South, Range 95 West, 6th P.M., Garfield County,
Colorado, as more fully described in the Lease.
In support of this request we have enclosed a certified copy of the Statement of Change filed
with the Colorado Secretary of State April 22, 2016, evidencing the change of entity name from WPX
Energy Rocky Mountain, LLC to TEP Rocky Mountain LLC.
