Exhibit A <br />Statement of Work <br />WATER ACTIVITY NAME—Continental Dam Spillway Restoration Project <br />GRANT RECIPIENT — Santa Maria Reservoir Company <br />FUNDINC SOURCE —$962,750 Statewide Account and $51,000 Rio Grande Basin Account <br />GENERAL DESCRIPTION OF PROJECT <br />The Company provides irrigation water to approximately 70,000 acres in Rio Grande, Saguache, Conejos, and Alamosa <br />Counties, in the San Luis Valley. The Company owns and operates two reservoirs: Santa Maria and Continental. The <br />reservoirs are located in Hinsdale and Mineral Counties, about 20 miles southwest ofthe Town of Creede. <br />Continental Reservoir was constructed between 1925 and ] 928 with a design capacity of 27,000 AF; however, due to <br />seepage and spillway deterioration, for the past 20 years it has been limited by the State Engineer's Office to a storage <br />capacity of 15,000 AF. <br />TASKS <br />The objective of this Project is to rehabilitate the Continental dam and restore the full storage capacity of the reservoir. This <br />will be accomplished by rehabilitating the existing spillway and installing an internal seepage collection system. <br />REPORTING AND FINAL DELIVERABLE <br />Design & Construction Reporting: The applicant shall provide CWCB copies of: Permits, Design & Construction <br />Documents ; Construction Documentation (periodic construction progress reports, change orders, meeting notes, schedule <br />summaries), and As-Build Drawings. <br />General Reporting: The applicant shall provide the CWCB a progress report every 6 months, beginning from the date of <br />the executed contract until the construction begins. The progress report shall describe the completion or partial completion <br />of the statement of work leading up to the advertisement for bid and including a description of any major issues that have <br />occurred and any corrective action taken to address these issues. <br />Final Deliverable: At completion of the project, the applicant shall provide the CWCB a final report that summarizes the <br />project and documents the project. This report may contain photographs, summaries of ineetings and reports/studies. <br />PROJECT COST ESTIMATE <br />The Colorado Water Conservation Board approved a grant for $1,013,750 ($962,750 from the WSRA Statewide Account <br />and $51,000 from the WSRA Rio Grande Basin Account) to be used in conjunction with a CWCB Construction Fund loan <br />of $3,041,250. The total project cost is estimated to be $4,055,000.
