IA I <br /> January 12, 2012 <br /> The Consolidated Mutual Water Company <br /> Attn: Mr. Mike Queen <br /> 12700 W. 27th Avenue <br /> Lakewood, Colorado 80215 <br /> RE: Proposal for Inundation Mapping for Fairmount Reservoir and Maple Grove Reservoir. <br /> Dear Mr. Queen: <br /> We are pleased to present you a proposal to prepare dam breach inundation mapping for <br /> Fairmount and Maple Grove Reservoir. <br /> We have attached our fee summary and I have also included a brief explanation of each item in <br /> our fee schedule. <br /> Task 1 - Records Research - We will search your files and the Office of the State Engineer's <br /> files to obtain copies of existing dam breach models and inundation mapping. <br /> Task 2 - Breach Modeling - We will prepare a breach model in accordance with the latest <br /> Breach Guidelines published by the Colorado Dam Safety Branch. The level of analysis will be <br /> "Simple" as described in Table 1 of the Guidelines for both the breach modeling and flood <br /> inundation mapping. <br /> Task 3 - Flood Routing Model - The flood routing model and inundation mapping will be based <br /> on the USGS 10m DEM or other more accurate topographic mapping that may be available. <br /> The limits of the breach flood mapping shall extend from the Fairmount and Maple Grove Dam <br /> and Reservoirs to the location downstream where the breach flood impacts are negligible. The <br /> downstream extent is expected to be Clear Creek at approximately 1-70 and US Highway 58, <br /> but the modeling effort may require some additional modeling of Clear Creek to verify that there <br /> are no more impacts downstream. <br /> Task 4 - Flood Inundation Mapping - The inundation limits will be prepared in ESRI shapefile <br /> format in UTM coordinates (NAD 83, Zone 13 - meters) which shall be overlaid on base <br /> mapping that includes color aerial imagery (2009 or newer). The updated inundation map will <br /> be printed in color on a minimum sheet size of 11x17. The inundation extents will be clearly <br /> illustrated along with peak discharge estimates, peak flood wave arrival times and peak flood <br /> wave depths and velocities at critical locations. Roads, highways and critical infrastructure <br /> (schools, hospitals, fire stations, etc.) shall be annotated and any roads expected to be <br /> overtopped by the flood wave should be called out. The map shall include a legend noting the <br /> dam name and damid, a north arrow and graphic scale. <br /> A Preliminary Inundation Mapping Report will be submitted to the State Engineer's Office for <br /> review along with the inundation map and supporting documentation (including electronic copies <br /> of supporting models, spreadsheets, etc.). Once comments from the State Engineer's Office <br /> are addressed, the final deliverables will include two copies of a final version of the Inundation <br /> Mapping Report describing the inundation mapping analysis to be delivered to the State <br /> UT Consulting,Inc. • 1400 W 122nd Avenue-Suite 120 • Westminster CO 80234 • 303-457-0735 <br />
