Attached is our annual benefit summary that provides a summary of our Health, Life, Disability,
Spending Account and EAP Plans. I have also provided a one plan benefits summary that contains our
employer cost in red. Finally there is a Rate sheet that includes examples of how to calculate life and
disability premiums. Please feel free to contact me with any questions.
