Agricultural Water User Member: <br />A. Instructions: Please ensure all the information in this area is complete and <br />legible. We need to have accurate information to be able to contact you in case of <br />questions and to ensure that you or your organization is correctly identified as a member <br />of the program. If you have a facility or project (or anticipate the development of a new <br />facility or project) which will benefit from SPWRAP membership, you may wish to <br />describe the member entity as the project proponent. <br />(If you or your agricultural organization is a shareholder in a participating <br />member ditch company which provides the majority of your irrigation water, you may <br />need a separate SPWRAP membership if you have or plan to seek permits unique to your <br />facility or property. Conversely, all the ditch company facilities which are incidental to <br />the overall operation of their ditches and reservoirs are covered by its participation in the <br />program.) <br />B. Calculation of Annual Assessment: We have included instructions for each of <br />the line items on the face of the form. If you need additional information, please contact <br />us, see below. <br />The SPWRAP Member Units of one unit per 10 acres irrigated (Line 4) and the <br />assessment rate of $30 per member unit (Line #5) are benchmarks set by SPWRAP for <br />2007 for all Agricultural Members. <br />Invoices: If you need an invoice before preparing your check, please complete your <br />form, calculate the amount due and mail it to SPWRAP at the address at the bottom of the <br />form or fax it to 970 - 482 -1486. We will prepare an invoice and send it to you by mail or <br />fax. <br />Payment of Assessment (Due by Anril 15, 2007): Please make your check payable to <br />" SPWRAP" and mail it to the address at the bottom of the form. <br />Industrial Water User Member: <br />A.
