� <br />Exhibit A <br />Scone of Work <br />C_J <br />The Project consists of the Contractor distributing Resource Action Program kits that include water conservation devices (1 <br />showerhead, 1 kitchen aerator, 1 mini tape measure, a flow rate test bag, toilet leak detection tablets, interactive Cdrom game) <br />and teacher curriculum guide to 4`�', 5` and 6`�' graders throughout the state. Three thousand seventy six (3076) kits will be <br />purchased and delivered by the Contractor. The Contractor will notify the state coordinator of various or selected classroom <br />locations, number of students and proposed dates kits are to be delivered prior to delivery. <br />The goals of the Project are to: provide kits to each student in the 4"', 5`�' or 6`�' grade classroom that is selected; providing support <br />and training materials for participating teaches, and to monitor and evaluate the program. <br />The Contractor will evaluate participation, education gained, and water savings from the Project through the summation of <br />completed pre and post tests; and student surveys listing installation of devices and usage. <br />A Final Report shall be compiled upon completion. The Final Report shall include a summary of the Project, discussion of the <br />Project implementation, Project findings and water savings results based on the evaluation of student surveys. The Final Report <br />will not only conclude the Project but will serve as information to be shared with other Colorado communities. <br />2• Proiect Time of Performance <br />The Project will commence on December 1, 2000, or upon full and proper execution of this Contract, whichever occurs later. <br />The Project will be completed on or before June 30, 2001. However, the Project time of perfarmance may be extended subject to <br />the mutual agreement of the State and the Contractor. A written request for extension shall be submitted to the State by the <br />Contractor at least thirty (30) days prior to June 30, 2001, and shall include a full justification for the extension request. In order <br />to extend the time of performance for this contract, the parties must enter into a contract amendment. <br />3. Budeet <br />Resource Action Program Kit Production and Delivery $45,000 <br />Sununary of Project, Written Report, Draft and Final for review $5,000 <br />TOTAL $50,000.00 <br />4• Schedule of Pavments <br />a• Agreement: <br />$ 45,000 Payment to be made only upon receiving evidence of shipment <br />of the Resource Action Program kits to classrooms. The Contractor <br />will submit documentation of shipment prior to the release of funds. <br />II. $ 5,000 Final payment to be made only upon acceptance of Project <br />completion. The Contractor shall submit a final financial statement <br />documenting the expenditure of all funds for which payment has been <br />requested and a final narrative report on the Project implementation <br />and findings. <br />III. Total $50,000 <br />All requests for payment after the first payment will be initiated by the Contractor in accordance with the provisions in pazagraph <br />B of the main body of this Contract. <br />5• Contract Monitorin <br />The Department of Natural Resources, Colorado Water Conservation Board, will monitor the Project on an as-needed basis. <br />6• Renortine Schedule <br />The Contractor will submit financial and narrative status reports detailing Project progress and properly documenting all to-date <br />expenditures of Grant funds at the time each payment request is made, in accordance with the payment schedule. <br />
