TENURE AND ISStlANCE OF A NEW PERMIT
<br />A. Exairation at tf~e End of the Authorized Period. This permit wilf expire at midnight on 1 2131 12 0 3 6. Expiration
<br />shall occur by operatian of law and shall not require notice, any decision document, or any environmental analysis
<br />or other documentation.
<br />B. Minimum Use or Occuaancv of the Permit Area. Use or occupancy oP the permit area shali be exercised at
<br />least 120 dav_s each year, unless otherwise authorized in wrifing under additional terms of this permit.
<br />C. Noafication to Authorized Officer. ff the hofder desires issuance of a new pertniE after expiration, the holder
<br />shall nofify the authorized officer in writing not less than six (6) months pr9or to tfis ~xpirafion date of this permit.
<br />p, Conditions for issuance of a New Permit A# the exp'sration or termination af an existing permit, a new permit
<br />may be issued to the holder oF the previous permit or to a new holder subject to ihe following conditions:
<br />1. The authorized use is compatible with the land use ailocation in the Forest Land and Resource
<br />Management Plan.
<br />2. The permit area is being used for the purpases previousfy authorized.
<br />3. The permit area is being operated and maintained in accordance with the provisions of the permit.
<br />4. The holder has shown previous good faith comp[iance with the terms and canditions of aH prior or other
<br />existing permits, and has not engaged in any activity or transactian contrary to Federal contracts, permits
<br />laws, or regulatians.
<br />E. Discrekion of Forest 5ervice. Notv,rithstanding any prov'ssions of any prior or other permit, the authorized offcer
<br />may prescribe new terms, conditions, and stipulations when a new permit is issued. The decision whether ta
<br />issue a new permit to a holder or successor in interest }s at the absolute discretion of the Forest 5ervice.
<br />F. Constn.ictian. Any construction authorized by this permit rnay commence by N/A and shall be campleted hy
<br />NfA. If canstruction is not completed within the prescribed time, this perrni# may be revokeci or suspended.

<br />applicahle Fedesal, State, and IocaE {aws, regulations, and standards, including but notlirnited to, the Federal
<br />Water Pollution Gontrol Act, 33 U.S.G. 1251 et seq., the Resource Conservation and Recovery Act, 42 U.S.C.
<br />690'I et seq., the Corr~prehensive Environmental Response, Compensation, and Lfability Act, 42 U.S, C. 9601 et
<br />se~c., and other relevant environmental laws, as well as pubtic hea~th and safety faws and other laws relating to
<br />the siNng, construction, operation, and maintenance of any facility, improvement, or equfpmenf on the property.
<br />B. Plans. Plans for development, layout, construction, reconstruction, or alteration of fmprovements on the
<br />permit area, as welf as revisions o~ such pfans, must be prepared by a qualified individual acceptable to the
<br />authorized officer and sha11 be approved in writing prior to commencement of wark. The holder may be required
<br />to furnish as-built plans, maps, or surveys, or other similar information, upon carnpletion of construction.
<br />C. Maintenance. The holder sf~all maintain the improvements and permit area to standards of repair, orderfiness,
<br />neatness, sanitation, and safety acceptable to the authorized officer and consistent with other provisions of this
<br />author¢ation. lf requested, the holder shall corrtply wiih inspection requlrements deemed appropriate by the
<br />au~horized afficer.
<br />b. Hazard Analysis. The holder hes a confinuing responsibility to i~entify all hazardous conditions on the permit
<br />area which would affect the improvements, resources, or pose a risk of injury ta individuals. Any non-emergency
<br />actions to abate such hazards shall be performed after consultation with the authorized officer. In emergency
<br />situa#i~ns, fhe holder shall notify the authorized officer of its actions as soon as possible, but not more than 48
<br />hours, after such actions have heen taken.
<br />~. Chan e of Address The,holder shall immediately notify the au~orized ofFicer of a change in address.
