Given the <br />diverse range ofpotential applicants, not all of the following information may be relevant. Where applicable <br />and relevant the description should include the following: <br />a? Type of organization, official Warne, the year formed, and the statutes under which the entity was <br />formed, a contact person and that person's position or title, address and phone number. For private <br />entities, a copy of the Articles of Incorporation and By-laws should be appended to the application. <br />The Colorado Corn Growers Association is a member associafion structured as a 501 c5. It was <br />formed in 1979 by corn producers in the State of Colorado, as the name implies. For purposes <br />of this application, the contact person is Mark Sponsler, CEO, 127 22nd Street, Greeley, <br />Colorado 80631, (970) 351-8201. Organizational by-laws will be attached separately. <br />b) For waters suppliers, information regarding the number of customers, taps, service area, and current <br />water usage, and future growth plans, water related facilities owned or used, funding/revenue sources <br />(existing service charges, tap fees, share assessments, etc.), the number of members or shareholders <br />and shares of stock outstanding or a description of other means of ownership. <br />The Colorado Corn Growers Association, the team leader and applicant for this grant, is not a <br />water supplier and does not own water related facilities. However, its membership represents a <br />major portion of the 1.2 million irrigated acres planted to corn in Colorado in 2008. It is difficult <br />to analyze the amount of water resources owned or leased by producers of this commodity, but it <br />is reasonable to estimate that the amount of water owned or leased by corn producers is one of <br />the largest classifications of the resource by volume. <br />c) For other entities, background, organizational size, staffing and budget, and funding related to water <br />that is relevant in determining whether the applicant has the ability to accomplish the program/project <br />for which funding is sought. <br />The Colorado Corn Growers Association has significant history and experience in grant <br />administration, particularly in projects that involve collaboration with other vested industry <br />partners. <br />1Vlost recent examples include: <br />?
