<br />b) For waters suppliers, information regarding the number of customers, taps, service area, and
<br />current water usage, and future growth plans, water related facilities awned or used,
<br />funding/revenue sources {existing service charges, tap fees, share assessments, etc.), the number
<br />of members or shareholders and shares of stock outstanding or a description of other means of
<br />ownership.

<br />water that is relevant in determining whether the applicant has the ability to accomplish the
<br />program/project for which funding is sought.
<br />The Lower Ark District was formed in 2002 by approximately 2:1 vote of the electorate
<br />of Pueblo, Otero, Crowley, Bent and Prowers Counties to conserve water resources for
<br />their greatest beneficial use within the District. The LAVWCD encompasses most of
<br />the Lower Arkansas River Basin, from above Pueblo Reservoir to the Kansas state line,
<br />including Pueblo and John Martin Reservoirs.
<br />The District has a general fund budget of approximately $1.7 million per year, funded
<br />primarily by a 1.5 mill levy on real property within the District. All of the budget is spent
<br />on water related activities, as described in mare detail in (d) below.
<br />The full-time staff of four includes the General Manager, Jay Winner, Project Manager,
<br />Bill Hancock, Financial Manager Kim Chavez, and Office Manager Carla Quezada. In
<br />addition, the District extensively uses the services of ifs outside General Counsel, Bart
<br />Mendenhall, and Special Counsel, Peter Nichols and Trout, Raley, Montano, Witwer &
<br />Freeman, P.C., as well as the services of consulting engineers such as HDR, Brown &
<br />Caldwell and Applegate.
