Funding is through purchase order OE PDA 05000000065 to our <br />financial "umbrella" the San Luis Valley Resource Conservation and Development Council. <br />Enclosed is a final report describing the project. Also enclosed are invoices that we have <br />accumulated since our last request in 2005, a listing and description of all invoices for the <br />project, and an accounting statement. The invoices do include costs to install a bronze plaque on <br />the culvert naming the CWCB and CWPF. The company needs a month to cast the plaque <br />(design photo below), but we will install the plaque on the culvert headwalls as soon as it comes <br />in. We also purchased native grass seed which will be spread on the reconstructed downstream <br />banks this spring. New invoices total $5,609.14, and we are requesting reimbursement for this <br />amount. With this amount, we spent a total of $41,662.29; less than the P.O. amount of $46,000. <br />The reimbursement should be sent to the SLV RC&D, but they have changed their address from <br />that listed on the P.O. to: <br />San Luis Valley RC & D <br />101 South Craft Drive <br />Alamosa, CO 81101 <br />Reimbursement Requested: $5,609.14 <br />P.O.
