Reporting to Municipality <br /> <br />Any permitted facility discharging to a municipal storm sewer shall provide the municipality with a copy of the pernrit application, <br />upon request. A copy of the SWMP shall also be provided to the municipality upon request. <br /> <br />8. Special Monitoring <br /> <br />a. IDS monitoring is only required for facilities discharging in the Colorado River Basin. IDS may be measured as electrical <br />conductivi~y where a satisfa:ctory correlation with IDS has been established based upon a minimum offive (5) samples. See <br />the individual Certification Rationale for a determination as to whether monitoring is required. <br /> <br />b. Phosphorus monitoring is required only for facilities which discharge into the following drainage basins: Cherry Creek <br />basin, Chadield Reservoir downstream of the USGS gage at Waterton and on Plwn Creek, Dillon Reservoir basin (i.e., Ten <br />Mile Creek" Snake River, Blue River, all tnbutaries to the Dillon Reservoir), and Bear Creek basin. See the individual <br />CertificatiOIl Rationale for a determination as to whether monitoring is required. <br /> <br />9. Si2Datorv Reauirements <br /> <br />a. All r<:ports required for submittal shall be signed and certified for accuracy by the permittee in accordance with the following <br />criteria: <br /> <br />1) In the: case of corporations, by a principal executive officer of at least the level of vice-president or his or her duly <br />authorized representative, if such representative is responsible for the overall operation of the facility from Which the <br />dischnge descnbed in the form originates; <br />2) In the case of a partnership, by a general partner; <br />3) In the case of a sole proprietorship, by the proprietor; <br />4) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranking elected <br />official, or other duly authorized employee. <br /> <br />b. Changes to authorization. If an authorization under paragraph a) of this section is no longer accurate because a different <br />individual or position has responsibility for the overall operation of the facility, a new authorization satisfying the <br />requirements of paragraph a). of this section must be submitted to the Division, prior to or together with any reports, <br />information, or applications to be signed by an authorized representative. <br /> <br />c. Certification. Any person signing a document under paragraph a) of this section (except for the SWMP certification) shall <br />make the following certification: <br /> <br />"I certify undc:r penalty of law that this document and all attachments were prepared under my direction or supervision in <br />accordance with a system designed to assure that qualified personnel properly gather and evaIuatethe information submitted <br />Based on my ilnquiry of the person or persons who manage the system, or those persons directly responsible for gathering the <br />information, the information submitted is, to the best of my knowledge and belief, true, accurate, and complete. lam aware <br />that there are significant penalties for submitting false information, including the possibility of fine and imprisonment for <br />knowing violations." <br /> <br />E. DEFINITIONS <br /> <br />1. A "continuous" measurement, for flow monitoring requirements, is a measurement obtained from an automatic recording device, <br />which continually measures flow. <br /> <br />2. A "grab" sample, for monitoring requirements, is a single "dip and take" sample. <br /> <br />3. An "in-situ" measurement, for monitoring requirements, is defined as a single reading, observation, or measurement taken in the field <br />at the point of discharge. <br /> <br />4. An "instantaneous" measurement, for monitoring requirements, is a single reading, observation, or measurement performed on site. <br />
