The percentages of minority and women-owned business participation will be
determined by dollar value of the work subcontracted to or joint ventured with minority
and women-owned firms, as compared to the total dollar value of the bid amount for all
work bid under this contract.

E. Prior to the award of this contract, the contractor will be required to provide to the
Principal Representative a list of W/MBE enterprises, stipulating the dollar amount of
each subcontract or supplier of materials on the Minority and Women Business
Enterprises Participation Report W/MBE-1 form..

F. A bidder who fails or refuses to complete and return Report W/MBE-1 shall not be
deemed a responsive bidder and will not be awarded the Contract. Where a bidder
intends to attain its goal for minority or women-owned enterprise participation by use of a
joint venture, the joint venture agreement and other appropriate evidence of percentage
participation must be furnished.

G. The awarding agency will review the contractor's minority and women-owned enterprise
utilization efforts during performance of this contract. If, at any point during the
performance of the contract, it becomes evident, or it materializes that a minority or
women-owned business enterprise listed in said contract is no longer available to
perform the services or supply the materials therein indicated, for whatever reason, the
contractor shall notify the Principal Representative of this circumstance prior to
subcontracting or otherwise engaging in the work specified. .

H. The contractor will retain records and documents showing the level of participation for
two years following completion of this contract. These records and documents, or
copies thereof, will be made available at reasonable times and places for inspection by
an authorized representative of the Principal Representative, or its designated
representatives, and will be submitted to such representatives upon written request.

MBEREPORT
Rev. 04/99
