<br />Grant Application - CWeR Construction Fund <br /> <br />Fonn Revised December 16, 1999 <br /> <br />Part C. - Project Sponsor Financial Information <br /> <br />The CWCB Construction Fund is primarily a revolving loan fund. Grants are approved only <br />when the project or study is of State interest and benefits a wide range of people. The following <br />information is requested to assist the CWCB in a preliminary assessment of the applicant's <br />fmancial capacity. It is also requested that the project sponsor submit with this application copies <br />of the two most recent annual reports, financial statements, corporate reports or other current <br />documentation of financial condition and operations. <br /> <br />1. List any existing liability or indebtedness that exceeds one thousand dollars. For example, <br />bank loans, government agency loans, bond issues, accounts payable, etc. Include names <br />and addresses of lenders, amounts, due dates and maturity dates. (You may attach a <br />separate schedule if you wish): <br />Neither NWCCOG, nor the Project, has debt. <br /> <br />2. Are any of the above liabilities now in default, or been in default at any time in the past? <br />YES NO If "YES", please give detailed explanation: N/A <br /> <br />3. Please provide a summary of all revenues received during the last two fiscal years by <br />category (such as service charges, tap fees, assessments, etc.): NI A <br /> <br />4. Please provide a brief narrative description of potential sources of funding (in addition to <br />the CWCB) which have been explored or which will be explored for the proposed project <br />or study. (Examples would be Local County and Town Governments, Water Conservancy <br />Districts, USDA Rural Development, The Natural Resources Conservation Service, The <br />U.S. Environmental Protection Agency, Commercial Banks, etc.) <br />As mentioned above, the financial sponsors have contributed $120,000 and the <br /> <br />5 <br />
