The April 19 LOMR revised the
effective Flood Insurance Rate Map (FIRM) and Flood Insurance Study (PIS) report for your community
to show the effects of the Tom Frost Reservoir project along Gay Reservoir Channel from the confluence
with West Lake Channel to approximately 2,300 feet upstream of Lowell Boulevard. Because the effective
date of that LOMR was later then the effective date (August 18, 2004) of the new countywide FIRM and
FIS report for your community, which was being processed at the time of the revision request, the updated
map panel numbers, profile panel numbers, and vertical datum were used for the annotated FIRM and FIS
report included with the April 19 LOMR. However, the community name and FIRM panel numbers were
incorrectly shown on the Determination Document and annotated FIRM panels issued for the April 19
LOMR. The purpose of this LOMR is to correct the community name and FIRM panel numbers shown on
the Determination Document and annotated FIRM panels of the April 19 LOMR.

We have revised the Determination Document and FIRM panels from the April 19 LOMR to reflect the
correct community name and FIRM panel number. The flood hazard information shown on the annotated
FIRM panels, profile panels, and Floodway Data Table enclosed with the April 19 LOMR has not
changed. The corrected community name and FIRM panel numbers are shown on the enclosed
Determination Document and annotated copies of FIRM Panels 0091 F and 0092 F. For your reference,
we also have enclosed annotated copies of Profile Panels 16P and 17P and affected portions of the
Floodway Data Table that were transmitted with the April 19 LOMR. This LOMR is effective as of the
date shown above.

A review of the determination made by this LOMR and any requests to alter this determination should be
made within 30 days. Any request to alter the determination must be based on scientific or technical data.

Because this LOMR will not be printed and distributed to primary users, such as local insurance agents and
mortgage lenders, your community will serve as a repository for these new data. We encourage you to
disseminate the information reflected by this LOMR throughout the community, so that interested persons,
such as property owners, local insurance agents, and mortgage lenders, may benefit from the information.
We also encourage you to prepare a related article for publication in your community's local newspaper.
