Application Summary

This is an application for surface water rights, storage rights, and approval of a plan for
augmentation in San Miguel County, approximately 5 miles southwest of Telluride. Applicant
seeks the following:

1. Surface water rights for points of diversions: POD #1, POD #2W, POD #3E, and POD
Spring under a July 2006 appropriation for I cfs (conditional) direct flow and 10 AF of
storage.

2. Storage water rights for Pond # 1, Pond #2 and Augmentation Pond for a total of lOAF,
filled (and refilled) at a rate of 1 cfs. The claimed uses include fish habitat and rearing,
livestock, recreation, wildlife, aesthetics, fire protection, domestic and domestic lawn and
garden watering, and augmentation. The three ponds will be used to supply water to two
residences.

3. Approval of a plan for augmentation using a portion of the water stored in the three ponds as
the sole augmentation source.

All structures are tributary to Gold Creek, tributary to the South Fork San Miguel River.
