The objectives
of this work were to evaluate the present condition of the dam, develop our own professional
opinion of the condition of the structure and the need for repairs, and to formulate an appropriate
work plan that will economically resolve the dam safety concerns identified by the Colorado
State Engineer's Office (SEO). The following paragraphs describe the scope of work performed,
our site inspection and evaluation of the present condition of the dam, review of existing data,
stability evaluations of the upper portion of the dam, and our conclusions and recommendations.
Our evaluations and analyses have focused on the upper concrete portion of the dam, since this
section of the dam is identified as being of greatest concern to the SEO.

SCOPE OF WORK

The scope of work for this evaluation consisted of the following work tasks:

1. Site inspection of dam, to visually evaluate the present condition of the dam.

2. Data review, comprising a review of available records and previous safety inspection
and engineering reports obtained from the DeWeese-Dye Ditch and Reservoir
Company, the SEO Division Engineer's office, and GEl files.

3. Stability evaluation of the upper concrete portion of the dam, using two-dimensional
gravity analysis methods.

4. Recommendations for future design and construction activities necessary to comply
with the requirements of the SEO.

5.
